VISUAL GUIDE 3 / 3

Turn a label into something the clinician can use

Instead of assuming…Describe the observation
“Needs stronger glasses”Cannot see the classroom board with current glasses on; noticed this month.
“Reads strangely”Closes one eye or moves unusually close during a particular task.
“Must be myopia”Describe distance blur, near difficulty, discomfort and when it occurs. Ask what assessment is needed.

Examples of wording, not fictional patient findings or diagnoses. Bring current correction and relevant records if available, but do not delay urgent care to prepare them.

A short factual note helps the appointment. The family does not need to identify the diagnosis before asking for help.Sources: [1] [3] [5] · Open or save this visual ↗
